Incentivate Health tracks data about all visitors and customers who use our Services and we log this information for and we will store this information. These logs may contain IP address information, types of operating system you use, the date and time you visited the site, the features you use, the links you click on, search terms and files you view.
We may also collect information about your computer, phone, tablet or other devices you use to access the Services. This device information includes your connection type and settings when you install, access, update, or use our Services. We do not knowingly allow Incentivate Health members under age 13 to create accounts for the Service. Only a parent or guardian may use the Service on behalf of a child under the age of 13.
